Transcripts and immunizations for current students are free.
Apply to a college FIRST before requesting to have your transcript sent to the college.

Graduates or former students of PHS Transcripts
Transcripts are $7.00 if ordered online ($5.00 transcript fee + $2.00 card transaction fee) by filling out the form below. Or you may come in to the school finance office and pay with cash for $5.00. If the transcript should be sent to more than one institution, the form must be completed again for multiple requests. Each transcript request is $7.00. Transcripts will be sent in 2-5 business days.

- Click on Sign in to create an account.
- Once logged in, select Nebo-Payson High School for the school.
- Select Public Items, and choose Transcripts. Click Add to Cart.
- Choose Continue to checkout, then proceed with your online payment.
- Make sure to fill out the form below!
